Please upload documentation demonstrating ownership of the property where the proposed accessibility improvements will be implemented. Acceptable forms of proof include:

1. A title deed vested in the name of the applicant organization;

2. An executed Purchase and Sale Agreement, signed by the applicant; or

3. another legally recognized instrument evidencing ownership by the applicant entity

Proof of Ownership

For Applicants who do not own the property where proposed accessibility improvements are to be implemented, submission of a fully executed long-term lease agreement evidencing lawful site control is required at the time of application. Additionally, if the application is conditionally approved, disbursement of grant funds shall be contingent upon the submission of a fully executed Owner Participation Agreement. This agreement will be provided as part of the conditional award notification and must be signed by both the property owner and Brilliant Corners prior to any release of funds.
